Call or use our automated system to schedule a visit. We offer same-day, evening, and weekend appointments.

We utilize easy-to-use video technology so you and your doctor can see each other securely, anywhere you are. Connect by phone, computer, or tablet. No apps required.
Monitor Your Progress
After your visit, take your medications as prescribed. You will get a connected blood pressure cuff to send your BP readings to us. Our doctors will review the readings and change your medications when needed.

Remote Blood Pressure Monitoring.
What is it?
Remote patient monitoring, or RPM, is the use of technology to monitor a patient's health data between visits. Data we can monitor include blood pressure, weight, blood sugar, oxygen level, and/or other parameters.
How does it work?
We will give you a blood pressure device to securely send your blood pressures to your doctor in near real-time. A 24/7 health operations center will monitor your blood pressures and let your doctor know when interventions are needed. Better yet, the service is fully covered by most insurance plans and, when covered by your plan, the first device is included at no cost to you.
How can it help me?
Research has shown remote blood pressure monitoring can lower your blood pressure significantly compared to usual, in-office care alone. RPM empowers patients to participate in their care and better manage their health.
Are there any other options?
Yes. You can check your blood pressure at home with a validated blood pressure device, and report the readings back to your doctor. Other tests such as ambulatory blood pressure monitoring, or ABPM, are available and offered at the clinic.
